Does anyone know a recipe for hot chocolate in the crock pot? ?

I had this recipe and I lost it. I know it had cocoa, sugar, milk and white chocolate chips, but I don't remember the rest. You put it in the crock pot for 2 hours. My kids are begging for me to make it. Please help!!!
